Rinnai error code 12 indicates a flame failure or lost flame fault. The heater detected that ignition occurred or should have occurred, but the flame signal was not present or disappeared during operation. This code is tied to the flame rod and flame detection circuit. Rinnai’s diagnostic guidance points to issues such as a bad connection at the flame rod, a damaged or contaminated flame rod, low inlet gas pressure, a defective gas valve, a faulty PC board, improper venting, or electrical grounding and polarity problems. The heater shuts down to protect itself when it cannot confirm stable combustion.
